When patients are not immediately transferred to post-acute rehabilitation or need a professional assessment during an interim of care, CNS can arrange evaluations wherever they are—whether in the hospital, an acute care setting, at home, or elsewhere. Individuals who have experienced brain injury may be discharged to different care settings after stabilization, or may suspect a brain injury and seek an assessment to determine the best next steps. CNS evaluators can meet patients in their current environment, providing essential insights to inform patients and their families of available care options.
Benefits of a Flexible, On-Site Evaluation Include:
Professional assessment by an experienced CNS evaluator in the patient’s current setting
Family and support members may be present to provide valuable insights on symptoms and changes they’ve observed
No-cost travel by CNS evaluators to wherever the patient resides, removing barriers to care
Insights into the patient’s current environment and support system that inform personalized treatment planning
Valuable input from loved ones and support members regarding patient memory, balance, pain, medications, emotional status, and sleep patterns
Other contributors, like attorneys or conservators, can be present to add relevant information
A comprehensive chronology of events can be developed to guide treatment planning and future steps
The assessment process includes questions on medical history, post-injury treatments, and pre-existing conditions, such as diabetes and hypertension. Additionally, an in-depth evaluation of medical and neurological factors is conducted, covering cognition, memory, language, motor skills, balance, and psycho-social elements. Patients and families can also gain insight into insurance coverage for brain injury rehabilitation.
Evaluations, lasting several hours, are followed by timely results shared within days and, upon request, can be provided to both patients and their families. For medically complex cases, CNS may also engage its specialized medical review team, which includes physicians, nurses, and neurologists, to ensure a thorough and multi-faceted approach.
