Sport Concussion Assessment Tool 5 (SCAT5)

The SCAT5 is a standardized concussion evaluation tool designed for licensed healthcare professionals assessing individuals aged 13 and older for suspected concussion. It combines on-field assessment for red flags and observable signs with a comprehensive clinical evaluation of symptoms, cognition, balance, and neurological function.

Recommended for use by medical professionals only, the SCAT5 supports evidence-based decision-making around diagnosis, management, and return to activity.

Healthcare professionals can access the official online SCAT5 tool here:

For all others, including coaches, parents, and non-clinical personnel, we recommend referring to the printable PDF for educational awareness:

Note: The SCAT5 is not a substitute for medical diagnosis. If you suspect a concussion, seek professional medical evaluation.
